Question
I am 22 years old , with pcos.....i take
metiformin and insulin...now for the past 3 mths i felt pregnant and i
have gotten really big...the pregnancy tests all say negitive so they
want me to take a pelvic examination. what are my chances of being
pregnant

|
|
Answer: You can do a blood test - beta hcg. This would confirm
ehether you are pregnant or not. Have an ultrasound done to rule out the
presence of an ovarian cyst or a fibroid in the uterus. |

Question
Doctor, I was having regular period initially till
20 yr of age but at a sudden I missed for 3 months, after a diagnosis, I
was suggested Novelon. I took novelon for 6 yrs and had no problems. Now
I am 26 yrs and my weight is 60 kg with my height 5.0 feet. I am married
for 2 yrs, stopped novelon for 7 months and missed period for 3 months
and after that I am spotting for 1-2 days for 2 months. My Gyn suggested
me the following tests:(Undergone on 2nd day of spotting) 1.Pelvis
examination:Multiple follicles of size 3-4 mm 2.FSH and LH : 4.15 and
1.52 mIU/mL 3.T3,T4,TSH : 3.14 pg/mL,0.78 ng/dL,2.420 uIU/mL
4.Testosterone : 1.06 ng/mL 5.Estradiol,E2 : 13.71 pg/mL 6.SHBG : 31.30
nmol/L 7.Prolactin : 77.42 ng/mL 8.DHEA : 10.7 ng/mL Now, after
reviewing the reports my Gyn suggested me for MRI scan with chances of
pituitary adenoma and also PCOS. 1.I don't want to conceive this time
and want to postponed for 2 yrs.Do you see that it will further create
problems? 2.What is the cause of this much
|
|
|
Answer: You have not mentioned the ranges of the tests. Each lab
has a different range , so I cannot tell you which is value is normal or
not. However, your prolactin must be hightand that is why an MRI has
been suggested. Has any medication like parlodel or dostinex been
advised to lower the prolactin levels ? Since you have PCOS , you would
have irregular periods and infrequent ovulation. You have not mentioned
your present age .If you are 25 and above, I suggest you have one baby
and then can wait for a second one , if you wish to have more than one
child. The reason is that you may take some time to conceive , in case
you are not ovulating. This can be determined by doing an ultrasound on
day 12 of the period. PCOs is present in 10-20 % of women .. |

Question
Hi doctor, gud evening. i got married by last
sep'08. my period went ahead for ten days on feb'09 and i thought i am
pregnant coz my periods were normal always. But, the test was negative
and doctor said i have PCOS. I was weighing 86kgs at that time. Did some
dieting and reduced weight to 82.5kgs. Now again, last month i got
periods for only two days. went to doctor and she did a ultrasound and
found small cysts. prescribed Metformin 500mg and i am takin the
medicine from last week. Today (11-8-09) s the 14th day and v did a
ultrasound and found that dominant follicles were not found. She asked
me visit her immediately after my next periods. Is that possible for me
to ovulate and get pregnant soon as v r expecting for a baby asap. I am
planning to start dieting and exercise again to reduce my weight. Please
reply.

|
|
Answer: PCO is associated with irregular periods and infrequent
ovulation. Losing weight would help to a certain extent. However, have
an ultrasound done on day 12 of the cycle to determine whether you are
ovulating or not. If not, you would require some medication like clomid
in the next cycle. Also, have your husband's semen checked . All the
best ! |

Question
Hi i have pcos i was given noretone by my doctor
to take to regulate my periods they lasted about three days and thet
were really slow i wore a panty shield for those days and sometimes they
be clean only when i urnie then i would see blood but know my breast are
swore and heavy itook a pregnant test it was negative what could be the
problem
|
|
|
Answer: I am not aware of the contents of noretone. However,
certain drugs can give scanty periods, so you do not have to worry about
that. If the problem persists have your thyroid hormones checked, as an
abnormality in these can also give rise to menstrual disturbances. |

Question
Im a 32 yr old woman trying to conceive for last 3
years. This year in June I had a missd miscarriage.I was taking folic
acid tabs and letroz 2.5 mg.After 3 months,I'm trying to conceive again
with the help of the same medicines. Is letroz safe? Please help.Thank You
|
|
|
Answer: You have to monitor your ovulation by ultrasound to
determine whether you are ovulating or not with letrozole. That would
also let you know your most fertile period. You must not keep talking
letrozole without monitoring as any of these drugs can cause cysts of
the ovaries which can cause problems. letrozole is not approved for
ovulation induction as it was believed to increase the risk of
abnormalities in the fetus. However, recent studies have not
demonstrated any increased risk. Also, were any reasons found for the
miscarriage ? Ensure that your blood sugar and thyroid hormones are
normal. |

Question
Hello Doctor, I am married for 2 years now. My
wife is taking treatment for infertility for 1.5 years now. In earlier
scans her egg size was not ok even with litroz. Then doctor changed the
tablet to fertomid and her egg size was 29mm and then doctor did IUI
which was not successful. In next Monthly period doctor suggested to
take fertomid again for 5 days starting from 3rd day. By mistake my wife
took 2 fertomid in a single day. This time egg was too large and even
reached 45mm and it is still the same on 2nd day of next period. then
doctor suggested novelon for 21 days from 2nd day. Please suggest
whether we are on the right track or not. Also there is continous
discharge from her left breast but doctor saying not to worry she took
carbegolin for 2 years twice a week but discharge is not getting
stopped. This discharge is from last 4 years. Her all reports related to
this are OK and carbegolin has been stopped. She is taking hyponidd also
twice a day. Thanks for you valuable reply. Regards,
|
|
|
Answer: A cyst can form even with a low dose of the drugs ( like
fertomid )like clomid, so you do not have to worry about that now, if
the cyst has reduced in size. Have a scan done on the 5th day of the
period to ensure that the cyst has reduced or disappeared. She must do a
prolactin level every 2-3 months. If the level is high she would need
cabergolin. If it is normal . she does not need it , even in the
presence of breast secretion. Close monitoring would be required during
every cycle to ensure when she is ovulating and that she does not form a
cyst again. You can try IUI for 3-4 cycles. You have not mentioned your
semen report and also whether her fallopian tubes have been tested and
found to be normal. Also ensure that her thyroid levels are normal. All
the best ! |

Question
Hi i had laproscopy and d&c done on 2nd sep this
year. I had periods after 2 days. last month i.e on 4th sep i should
have periods, but i got them on 16th oct.I have been using leuporodile
3.75 mg for 3 months. is it common if i have them as i heard periods
won't come bcoz of leuprolide. doc said after 3 months of using it u try
fertil tablet for 5 consecutive days for next month. will i get cyst and
endometriosis for these 3 months. i am worried bcoz i have to concieve
soon.
|
|
|
Answer: Leuprodile could delay your period. However, in some
women irregular bleeding can occur, so you do not have to worry about
that. Did you have a cyst on laparoscopy ? Has the size of the cyst
reduced after the three doses ? Endometriosis does tend to come back .
so you should try and conceive as soon as you get a period after the
last injection. Careful monitoring by ultrasound should be carried out
after using fertil, to ensure that you are not forming cysts again. All
the best ! |

Question
Dear Dr. I had concenived with IUI during my first
pregnancy and now my baby is 2 years old. I have had a problem of
irregular periods. IUI was done after 1 year of trials through natural
way. I was given a medicine called siphene after which i ovaluated
normally. now my kid is still breast feeding and i want to conceive
again. my follicular monitoring shows that the folliciles and not
increasing in size but remain at size 6 or 7 mm. my dr says that since i
had problems before also so there is little chance regluar ovaluation
even with medicine if i still keep feeding my child. my child is very
fussy and just not responding to my weaning efforts. is it really not
possible to ovaluate normally while feeding even if i take medicines.??
also will these medicines affect my child since she is feeding on me.
|
|
|
Answer: Lactation ( breast feeding ) may prevent ovulation. Thus,
you must stop lactation and then try to conceive. You may ovulate even
while lactating. In any case, speak to your pediatrician and take advice
regarding weaning. Even if you do conceive while breast feeding , you
would need to stop once you are pregnant, so make that effort to do it
now. . In the meantime, however, you can start folic acid tablets. |

Question
Dear dr. I have an 8 mth baby and iam
breastfeeding. Im trying to concieve again but Iam not ovulating due to
breastfeeding, which i dont want to stop. Is there any injection I can
have to start my ovulation? Any other suggestions? Thank you, Michelle
|
|
|
Answer: Lactation ( breast feeding ) may prevent ovulation. Thus,
you must stop lactation and then try to conceive. You may ovulate even
while lactating, but the chances are less. You can have an ultrasound
done od day 12 of the period to determine whether you are ovulating or
not.Tablets or injections maybe ineffective while you are breastfeeding.
Even if you do conceive while breast feeding , you would need to stop
once you are pregnant. |

thrice a day, susten 400 inserted twice a day & folic acid once a day. i
did scan in week 5 & foetus was in the uterus heartbeat 105. i have

stains the pant but not a lot to wear a pad. My Dr. asked me to take
sylate for 5 days which ended yesterday. the discharge has not stopped.
today morning i started spotting again. Dr. said i take primulut 250mg
injection & bed rest for 2 days. What is causing the spotting? is the
baby safe? any precautions to take?
|
|
|
Answer: You must have an ultrasound done to ensure that the baby
is ok.There are many causes for bleeding like chromosomal abnormalities,
thyroid disorders, diabetes, toxoplasmosis infection ( and others ).
please ensure that you do not have any of these. Chromosomal
abnormalities , however, would be difficult to detect at present by any
test. Fibroids can also cause bleeding. All the best ! |

Question
I Have'nt started my period I should have started
on the Nov 9. I took a HPT and came back negative. I feel like I Have a
urinary tract infection can that cause my period to delay. and also will
the HPT work if i Have an infection
|
|
|
Answer: The pregnancy test would come positive around 15 days
after conception. So, if you have ovulated late, the test would be
positive later. The infection may delay the period, specially if you
have taken certain medication for it. Urinary infection would not affect
the result of the test. |

Question
I am 27year old.1 year completed after my
marriage.I have irregular periods.Usually I get in periods 40days .This
time delayed to 50 days ,still no periods. I took home test and it was
negative. I went to doctor and had a blood test. The result say my HCG
level is 1.76 mIU. I am confused , Am I still pregnant or not? Please
let me know your suggestion.
|
|
|
Answer: The HCG is below the level for pregnancy. Only if it is
above 4 or 5 there is a pregnancy. If you are wishing to conceive, start
taking folic acid. Also, have a scan done on day 12 of the period to
determine whether you are ovulating or not and which is your fertile
period. |

Question
Hi Doc, I had my period on 14th October. Took
postinor on 23rd October. Started bleeding 29th October until today, 8
th November. The bleeding is not much, but red in colour. My cycle is 25
days. I have a small fibroid. I am 42 years old and 4 children. What
should I do? SHould I wait until the bleeding stop by itself?
|
|
|
Answer: You must have stopped bleeding by now. If your scan had
been done before 3-4 months, repeat it to determine whether your fibroid
has increased and whether you have an ovarian cyst. Both of these can
cause prolonged bleeding. |
